Dental implant pain: what to expect
Getting a dental implant is one of the most feared procedures… often needlessly. Performed under local anaesthesia, it is painless during surgery. Afterwards, discomfort is usually mild and lasts a few days, comparable to a tooth extraction. Here is what you will really feel, step by step, and how to manage it.
During surgery
No pain. Local anaesthesia (or conscious sedation) fully numbs the area. You feel at most vibrations and light pressure.
Days 1 to 3
Moderate discomfort, mild cheek swelling, sometimes a small bruise. This is the peak, easily controlled by prescribed painkillers.
Days 4 to 7
Pain fades quickly. Most patients stop painkillers and resume normal life, including work.
After 1 week
No pain at all in the vast majority of cases. The silent osseointegration phase (3-6 months) then begins, completely painless.
Does placing an implant hurt?
No. Implant surgery is performed under local anaesthesia, just like a routine dental treatment: you are awake but feel no pain, at most some pressure. For anxious patients or longer procedures, conscious sedation can be offered for total comfort.
Pain only appears afterwards, once the anaesthesia wears off (2-4 h), and stays mild to moderate in the vast majority of cases.
How long does implant pain last?
The typical recovery timeline:
No pain during surgery. In the evening, discomfort appears as anaesthesia fades: start painkillers and ice the cheek.
Swelling and tenderness peak around 48-72 h. Pain usually rated 3-4/10, well controlled by paracetamol ± ibuprofen. Soft, cold diet.
Swelling subsides, pain becomes minimal. Most patients stop painkillers and resume their activities.
Complete gum healing, no more pain. The implant enters silent osseointegration.
How to relieve pain after placement
💊 Painkillers
Paracetamol as a base, ibuprofen if needed (unless contraindicated). Follow your practitioner's prescription without exceeding doses.
🧊 Ice
Cold pack on the cheek in 15-minute cycles during the first 24-48 h to limit swelling.
🥣 Soft diet
Lukewarm/cold, soft foods (purées, yoghurt) for 2-3 days. Avoid heat, alcohol and tobacco.
🪥 Gentle hygiene
Prescribed mouthwash, gentle brushing. No spitting or strong suction in the first 48 hours.

Abnormal pain: when to see a dentist?
Pain that increases beyond 3-4 days instead of easing should raise a flag. Contact your practitioner if you have:
- Intense pain that worsens after day 3;
- Major swelling lasting beyond one week;
- Fever, pus discharge or persistent bad taste (sign of infection);
- An implant that moves (rare — risk of osseointegration failure).
These complications remain rare: the implant success rate exceeds 97%. Insufficient bone, heavy smoking or poor hygiene are the main risk factors — often anticipated with a bone assessment and graft if needed.

Does placing a dental implant hurt?
No. The procedure is performed under local anaesthesia and is painless. Pain only appears afterwards, once the anaesthesia wears off, and stays mild to moderate in the vast majority of cases.
How long does pain last after an implant?
Discomfort peaks between day 1 and day 3, then clearly subsides. Most patients no longer need painkillers after 4-5 days and feel nothing after one week.
Which painkiller should I take after a dental implant?
Paracetamol is the base; ibuprofen can be added unless contraindicated. Always follow your practitioner's prescription.
Is it normal for an implant to hurt weeks later?
No. Pain that persists or returns weeks later may signal peri-implantitis or osseointegration failure. See your practitioner promptly.
Are multiple implants (All-on-4/6) more painful?
They cause slightly more swelling than a single implant but remain well tolerated with an adapted painkiller protocol. Most patients resume normal activity within 5-7 days.
